automatic scoring questionnaire FORMS

The .pdf documents available for download will automatically score the SCAS questionnaire and help to calculate corresponding t-scores. The original version of these templates were developed by Abel Chapa ( M.Ed., LPC-S) and we are grateful to him for making these available to others. Please follow the steps below for download and scoring instructions.

The automatic scoring questionnaires are designed to be used from a file stored locally on your computer or laptop. They do not work on a tablet, mobile device or in PDF preview - it will not calculate or save. Also please do not try to enter the data directly onto the website when you open the file as it will not save the data nor compute. Please follow the instructions provided below.

9 Steps to DOWNLOADING AND USING THE AUTOMATIC SCORING FORM

*Please note the form does not save or calculate your data on this site or in PDF Preview. 
  • Download appropriate PDF file from links in sidebar
  • Save the file to the desktop of your computer, giving it a new file name before using it.
  • Make a new copy of the file for each client, making sure to press RESET on the back page to delete prior data. Name the new file and save it for that client ID.
  • The clinician enters the client's name, clinician name, date, file number etc. ( Free text boxes are also available to record both patient / clinician name, date of completion, file number and any other relevant clinical information such as specific phobias.)
  • The child then enters their data. ( Clicking the arrow on the corresponding item drop-down box to select the child's response. Highlighting and clicking the relevant response option selects the child's answer to each item.)
  • Check that all questions have been answered.
  • When the form has computed the scores for sub-scales and total score on the final page, you need to click on the pull-down menu in the next box to manually enter the score range. *Please note: you need to manually enter the score range, it is not automatic.
  • Then manually enter the T-score range into the last line in order to get the final level (normal range or elevated).
  • To save the results, press print and save as a .pdf file. 

Interpretation

T-Scores above 60 are considered elevated.
*Please note this tool is not intended to be used on its own as a diagnostic measure. Rather, it is designed for use either as a screening measure to identify those with elevated anxiety symptoms or for use in partnership with a structured clinical interview for diagnostic purposes. It also provides a valuable means of assessing change in anxiety symptoms over time.
